San Diego Crew Classic - Parent Authorization Form

All athletes participating in the Winter 2015-16 Season must submit this form. This is important information for the use of our coaching staff when making selections for the Boys 8 and the Girls 8 which will travel to compete in San Diego on April 1 through April 3.

You must answer the following truthfully. If an athlete is gone on vacation (spring break or other) between January 31 and departure on April 1 they will NOT be considered for one of the 8's competing in San Diego.
